Staining these structures can increase the efficiency of a nontraumatic removal.</p>
<p><strong>Methods:</strong> The surgeon performs a partial core vitrectomy and induces a posterior vitreous detachment. The vital dye is then injected into the retrohyaloid space in balanced salt solution (BSS). The dyes used are TWIN (AL.CHI.MI.A. Srl, Padova, Italy), MembraneBlue-Dual (DORC International, Zuidland, The Netherlands), and Doubledyne (Alfa Intes, Casoria, Italy). The surgeon can complete the vitrectomy and gradually aspirate the dye with a probe. Once the vitrectomy is complete, the surgeon can perform the peeling of the ERM without the need to reinject the vital dye over the macula.</p>
<p><strong>Results:</strong> The presence of the dye over the macula facilitates the visualization of the vitreous cortex by blocking the red reflex and increasing the contrast power of the coaxial light probe during the vitrectomy. This allows a negative staining of the vitreous because the dye acts by increasing the visibility of the surrounding BSS and not the vitreous itself.</p>
<p><strong>Conclusions:</strong> We describe a new chromovitrectomy technique using the same dye to increase the visualization of the vitreous, posterior hyaloid, ERM, and ILM.</p>

Diciannove coloranti, di cui tre di nuova sintesi, sono stati valutati per intensità e selettività di colorazione della membrana limitante interna (MLI) e facilità di eliminazione della colorazione durante la simulazione della vitrectomia, seguita da “ILM peeling”, in bulbi di maiale ex vivo. I coloranti, singoli o combinati con Trypan Blu (TB), risultati idonei alla colorazione sono stati testati per la citotossicità secondo ISO 10993-5. La combinazione dei coloranti selettivi e non citotossici è stata sottoposta a valutazione clinica nella chirurgia del pucker maculare. Solo due coloranti, Blulife e BBG250, hanno mostrato una colorazione selettiva per la MLI. Un colorante, Erioglaucine sale disodico, ha colorato sia la MLI sia la membrana ialoide (MI). Tre coloranti, Acid Violet 17, Brilliant Black and Direct Blu, hanno colorato selettivamente solo la MI. Acid Violet 17 è risultato citotossico alla concentrazione efficace. Blulife è risultato non citotossico alla concentrazione di 0,05% (p/v), mentre BBG250 è riportato come citotossico in letteratura alla concentrazione di 0,03%(p/v). L’utilizzo della miscela dei coloranti Blulife e TB nella chirurgia del pucker maculare ha confermato la visualizzazione delle membrane epiretiniche e la colorazione blu brillante, selettiva ed omogena della MLI. In particolare, nei pazienti diabetici, la colorazione selettiva della MLI ha permesso di evidenziare e successivamente rimuovere tutti i frammenti di MLI. La combinazione dei coloranti Blulife e Trypan blu permette una colorazione blu delle membrane epiretiniche e della MLI in maniera selettiva, intensa e sicura.</p>
